In addition to the registration forms, certain documentation is required to enroll in Minnesota public schools.

Immunization/Vaccine/Shot Records. The State of Minnesota requires students enrolled in child care & schools to provide proof of vaccination. If your child needs vaccines to meet these requirements, visit your family physician, clinic, or contact your county’s health department.  Families who seek exemption from these requirements may submit an Affidavit to Object Immunizations MN in place of immunization records to continue enrollment.

Birth Certificate. For students applying to Kindergarten, a birth certificate is required to prove the age of the student.

Early Childhood Screening.  In Minnesota, children are eligible to attend Kindergarten when they are five years old on or before September 1 of the calendar year that schools starts (Minn. Stat. § 120A.24).  Early Childhood Screening is required for entrance in Minnesota’s public schools or within 30 days of enrollment into Kindergarten. Early Childhood Screening is offered throughout the year by local school districts. Please call your local school district to schedule screening for your child.
